Memorial Donations

    Please download and use the form above when consulting with the patron considering or making the memorial donation.  Turn the form in to either Michael, Linda, or Mary Campbell.  Michael will contact the patron if more information or further clarification is required. 

    As soon as items meeting the request have been identified (and if need be, purchased), Acquisitions will write letters thanking the donors and informing the family members of the party for whom the memorial has been created that materials have been generously donation by the donors in honor or memorium of the noted party.
